1 NOTIFICATION OF BIRTH （Shussho-todoke）
Please submit a “notification of birth” to City Hall with 14 days from the date of your childbirth.
Please go to the Immigration Office to request for permission of residence within 30 days from the date of your childbirth.
Items to bring
Maternal and child health handbook
Residence cards of the child’s parents
Passports of the child’s parents
2 NOTIFICATION OF DEATH （Shibo-todoke）
Please submit a “notification of death” to City Hall within 7 days from the person’s death.
Please return the deceased person’s residence card to the Immigration Office via mail or other method.
Item to bring
The residence card of the person who makes the notification
